When you transfer a character to another server, the legacy on the destination server become a combination of both legacies (the one from the new server and the one from the old server), where any unlocks, achievements, etc. from either legacy are included in the combined legacy.

Transferring servers costs 1,000 Cartel Coins or approximately $10.00 USD, or about 2 months worth of subscription free cartel coins. Due to the high cost, it is not recommended to transfers multiple characters or to casually transfer servers. There is no server transfer item in-game - you pay the cost on the swtor.com website.

Your legacy storage does not transfer. Essentially, put everything you want to take with you in your personal storage, or on your character and then log out. Once in your new server, all achievements, levels, reputations and so on will be there. As you said, your decorations will be available, but not placed.

If you transfer a character to a new server, that character will take your Legacy with them to the destination server.

I have several characters on one fully levelled legacy. If I were to transfer one of those characters to a second server, it's my understanding that the server with the highest level merges with the one on the destination server (as the second legacy is a lower level)
